To access the form, click on the link below. A dialog box will pop-up asking "Do you want to open or save this file?" Click on the field to position the cursor inside the form field. The I-beam cursor allows you to type text. The pointing finger (or arrow) allows you to select the check boxes. After entering text or selecting a check box, choose from the following:
- Press Tab to accept the field change and go to the next field.
- Press Shift+Tab to accept the field change and go to the previous field.
- Press Return to enter multiple lines of text in the field; it does not accept the field change nor deselect the current field.
