MINIMUM
ORDER: |
$50.00 - freight charges are included;
additional charges may apply for, rush courier or overnight shipment
requests not to exceed the carrier's actual freight charges. Additional
HAZMAT fees may also apply. |

DELIVERY
TERMS: |
Delivery is FOB Destination to the
ordering entity’s specified address. Routine consumables delivered
within 48 hours ARO. All other equipment and supplies must be delivered
within 7 days ARO, unless a longer delivery time is agreed to by the
ordering entity. |

|
|
EXCLUSIONS:
|
(1) information technology software,
defined as any program or series of programs offered commercially to
computer installation, unless incidental to the operation of the equipment
being purchased; (2) consulting services; (3) items with a net discounted
individual unit acquisition cost greater than $25,000; and (4) equipment
and supplies which may be in the vendor’s catalog, but which are
not specifically designed or intended for laboratory use (e.g. reception
chairs, couches, coffee tables, etc.). |

CONTRACT PRICING: |
Discounts are firm throughout the term
of the contract. |
|
|
FUEL SURCHARGE |
A variable fuel surcharge has been
approved by NASPO/WSCA-ID for the referenced contract for orders placed
on or after 5/30/12. The surcharge for orders placed under this contract
only applies when diesel fuel for the prior month's average exceeds
$3.69/gallon in accordance with the U. S. Department of Energy at www.eia.gov/petroleum/gasdiesel
(Gulf Coast (PADD3). If fuel drops below $3.69, no fuel surcharge is
allowed. The surcharge is invoiced at the individual order level; consolidation
of orders is encouraged |

CONTRACT PERIOD: |
October 1, 2010 thru June 30, 2013
with two (2) optional renewals of one (1) year each. |
