Universal Service Fund Discount

Applying for Universal Service Fund Discount for LaNet Access According to the FCC Fourth Order on Reconsideration in CC Docket No. 96-45, LaNet is considered an Internet Service Provider and can provide access to the Internet for K12 and library organizations at USF discounted rates. Please follow the following procedure to ensure that your organization receives the proper discount for your connection to LaNet.
For organizations that DO NOT have a contract or service with LaNet that covers the Funding Period for which you are applying, the following procedures will apply. If you DO have a contract with LaNet, see procedures below.
- Organizations interested in receiving a bid from OTM/LaNet for Internet Services should forward a copy of the FCC Form 470 to the LaNet NOC, or notify LaNet by phone or email that a response is requested.
- Upon receipt of this request, LaNet will send a standard package of information which includes rates and line of service information to the organization. This will serve as LaNet's bid response.
- The organization will review the bids received and make a selection. If LaNet Access is selected, the organization will complete the LaNet Access Service Request form and send it to the LaNet NOC. The contract period is indicated on the form.
- LaNet will give the organization the specific information required to complete the FCC Form 471. (OTM/LaNet's SPIN number is 143004988.)
- Upon approval of USF discount, the SLD will send a copy of the Funding Commitment Letter to OTM/LaNet. OTM/LaNet will contact the organization and verify the Service start date.
- Installation of services to the organization will begin. Four to five weeks should be allowed for the installation process.
- Once the installation is complete and the organization is able to access the Internet, LaNet will notify the organization that billing will begin the first of the month following the installation date. The organization will be billed at their approved discounted rate.
For organizations that DO have a contract and service with LaNet that covers the Funding Period for which you are applying, the following procedures will apply:
- If the organization decides to continue service with OTM/LaNet, a form 470 should be filed with the SLD, even though a competitive bid is not required.
- If requested, LaNet will send the organization the specific information required to complete the FCC Form 471 (LaNet's SPIN number is 143004988).
- Upon approval of USF discount, the SLD will send a copy of the Funding Commitment Letter to OTM. OTM/LaNet will contact the organization and verify the funding approval. The organization will be billed at their approved discounted rate beginning the month following approval. Credits or refunds will be issued for previous service during the Funding Period, if applicable.
