Water Sector Program Forms

User Access Form - must be completed and approved in order for an entity to be given access to the portal. Water and sewer systems, as well as consultants/engineers, must complete the form in its entirety. Refer to the Instructions to Complete the Water Sector Program User Access Request Form and submit the completed User Access Form to Traci.Watts@la.gov for processing.

New - Request for Payment Form - Must be downloaded and saved to your computer and then reopened in Acrobat Reader where you will complete it electronically and add a digital signature. Submit by email to OCDLGA.PaymentRequests@la.gov.
